To apply for a UDID card online, use the official Swavlamban portal, submit your disability certificate application and follow the instructions of the hospital or medical authority assigned to it. Submitting the form is only the first stage. The authority verifies the application, arranges a specialist assessment where required and generates the disability certificate and Unique Disability Identity card after the medical decision.

Who can apply for a UDID card?

The official UDID FAQ says the portal incorporates the 21 specified disabilities under the Rights of Persons with Disabilities Act, 2016. People diagnosed with a specified disability may apply for certification. The medical authority determines the disability type, percentage and certificate validity through the prescribed assessment process. An applicant does not decide these entries by selecting a preferred percentage.

The card records disability identity and helps people access schemes, but it does not itself promise a pension, scholarship or a fixed cash payment. Each benefit has its own eligibility rules. The FAQ directs pension questions to the authority providing that pension. Someone holding a UDID card should therefore check the particular scheme's conditions before assuming that certification automatically qualifies them for its benefits.

UDID card documents required

The portal's document checklist includes a colour photograph, Aadhaar details, address proof and an existing disability certificate if the applicant already has one. A person who has an older certificate should select the relevant option in the application. The medical authority may still decide that reassessment is necessary; uploading a previous certificate does not remove that possibility.

For uploads, the FAQ specifies a photograph of 15-100 KB, signature of 3-30 KB, and address proof and disability certificate files of 10-500 KB each. Photograph and signature formats include JPEG, JPG, GIF and PNG; the document entries also allow PDF. Check the instructions displayed in the live form before submitting, especially if an upload is rejected or the portal changes its requirements.

How to apply online and attend the assessment

Start with the official Apply for UDID instructions. Read the workflow, complete the application carefully and retain the receipt. The FAQ says the acknowledgement identifies the assigned medical authority or hospital. Its details can also be checked through the PwD dashboard. Use that information when arranging the next stage rather than visiting an unrelated hospital.

When the application is in Submitted Mode, the published workflow tells applicants to visit the selected hospital's UDID or disability certification section and follow up on verification or assessment scheduling. The authority checks the application and may require corrections. After verification, specialist doctors assess the applicant and prepare their report for the medical board's decision.

The medical board determines the type and percentage of disability and validates the certificate. The concerned authority then generates the certificate and card on the portal. The Department of Empowerment of Persons with Disabilities dispatches the physical card by Speed Post. The application guide does not establish a universal date by which every applicant will receive an assessment appointment.

How to check UDID application status and download the card

Use the Track Your Application option on the UDID portal and enter the identifier requested there. Keep your enrolment or request details available. The FAQ distinguishes the online application status from your position in a hospital's assessment queue: for the latter, contact the assigned medical authority or the district CMO or DMO office.

After generation, the PwD dashboard provides the digital card and disability certificate. Temporary certificates require renewal when their validity expires; follow the renewal instructions and any assessment directions from the hospital. Corrections also depend on the processing stage. The FAQ allows application editing while it remains in Submitted Mode, before the hospital has acted on it.

Where to get help if the application is pending

Begin with the hospital or issuing medical authority shown against your application. The department's project page identifies district welfare officers as another source of assistance with registration. For unresolved grievances, the FAQ lists 011-24365019. These instructions were checked on 2 November 2026 at approximately 21:49 IST. No new nationwide application deadline or guaranteed processing time was confirmed.
